Registering

The practice welcomes new patients and we accept registrations if you live within the practice boundary. We encourage you to register as soon as you move into the area rather than wait until you are unwell, as records from your previous GP may take some time to be transferred to us.

You will need to fill in a form to register, and a questionnaire. We need the name and address of your previous GP so that your records can be traced.

If you move house it is important that you let us know your new address. If this address is outside our practice area you will be asked to register with another practice.

Please come in to any one of our sites to complete the paperwork, or download the forms below and bring them already completed to us. At the time of registration we do ask for photographic ID for all adults (passport or driving licence). This is to protect unauthorised people from gaining access to medical records.

We ask that patients give us 2 working days to process their registration requests prior to asking for an appointment.

It is also important that we have an up-to-date contact telephone number for you at all times, which we would prefer to be a land-line and not a mobile number.

Patients have a responsibility to keep us informed of their current address and phone number so that we can keep in touch.
